WellTide Health creates a collaborative health ecosystem to better enable Nova Scotians to be proactive participants in their health. We are committed to breaking down barriers to healthcare and working collaboratively with our tenant partners to lead the charge for new and better access.
Health Navigation
Health navigation is an integral part of our care delivery model. WellTied Health navigators will help patients access services and care they need. With personalized guidance through the process, patients can get the support they need to improve their health outcomes.
Proactive Emphasis
WellTide Health is designed to complement the primary care system by providing Nova Scotians with an avenue to be proactive with their health and avoid adding strain to the acute care system. WellTide Health will invite a community of like-minded tenant partners to thrive while serving patients take control of their health by participating in a multidisciplinary health ecosystem.
Technology Driven
WellTide Health will place a heavy emphasis on innovation and technology, removing some of the traditional silos within healthcare. Patients will access a personal portal and be active participants in the process. With their consent, true collaboration will be achieved when their records and health history are accessed by tenant partners working with the patient.

Own or operate a business providing health services, care and supports to patients
Demonstrate a proven, positive impact on the health and well-being of patients
Seek ways to leverage innovative approaches and leading-edge technology to deliver an exceptional patient experience
Believe patients benefit from being empowered to actively participate in their healthcare journey
Are looking for like-minded health providers and community partners to deliver holistic patient-centered care
Are actively engaged in their community and open to new service models to improve access to healthcare and improved health outcomes, regardless of socioeconomic status
