About the PFC Collaboration Hub

In rural Nova Scotia, many professionals find purpose in close-knit communities where career and lifestyle come together. With this in mind, Partners for Care is creating the PFC Hub.

The PFC Hub is a 5,000 sq. ft. space located on the second floor of the WellTide Health facility, that serves as the operational heart for PFC’s community health efforts - reflecting the realities, strengths and needs of the region it serves. The PFC Hub aims to address regional health, wellness, and social support gaps, such as limited primary care, allied health shortages, and service fragmentation, to support all aspects of residents’ health and social determinants of health.

How it benefits community members

Health Navigator

At the centre of the PFC Hub is the navigation service, acting as the community’s frontline point of contact to help people understand what services are available, how to reach them, and how service providers connect inside the PFC Hub and across the community. Navigators will welcome visitors, conduct interviews in person and on calls to understand needs, and help people navigate to the appropriate services.

They will work with the reception staff to identify patients in need of services. They will also maintain an up-to-date inventory of local health and social resources and make connections, with follow-up as needed to ensure success.

Accessibility

The PFC Hub will also incorporate a virtual care pod - a private, sound-isolated booth equipped with high-quality video conferencing hardware and secure Internet access that will connect people to primary care providers to provide symptom-based assessments and treatments.

The services offered in the PFC Hub will be built around health equity, ensuring everyone can access support regardless of income, background, or circumstance. Through the PFC Hub, WellTide Health becomes an accessible gateway that helps individuals find and connect with the right services efficiently, locally, and with dignity.
