About the PFC Hub

In rural Nova Scotia, many professionals find purpose in close-knit communities where career and lifestyle come together. You’re part of a supportive team, developing genuine relationships with colleagues and patients—people who value being recognized and remembered. With this in mind, Partners for Care is creating the PFC Hub.

The Partners for Care Collaboration Hub (PFC Hub) is a 5,000 sq. ft. space that will serve as the operational heart of WellTide Health, offering a modern co-working space for health and wellness providers to deliver care. Reflecting the realities, strengths, and needs of the region it serves, person-centred navigation will be a guide for the community. ​

How it benefits community members

On-Site Navigator

At the centre of the PFC Hub is the navigation service, acting as the community’s point of contact to help people understand what services are available, how to reach them, and how service providers connect inside the PFC Hub, within WellTide Health, and across the community. An on-site Navigator will conduct in-person and phone interviews to understand an individual’s unique needs. Then they will help them understand how to navigate to the appropriate services and resources that are important to their well-being.

The following is an example of a patient journey where the PFC Hub Navigator plays a key role in empowering a community member to connect with the resources that best serve their needs:

Accessibility

The PFC Hub will also incorporate a virtual care pod - a private, sound-isolated booth equipped with high-quality video conferencing hardware and secure Internet access that will connect people to primary care providers to provide symptom-based assessments and treatments.

The services offered in the PFC Hub will be built around health equity, ensuring everyone can access support regardless of income, background, or circumstance. Through the PFC Hub, WellTide Health becomes an accessible gateway that helps individuals find and connect with the right services efficiently, locally, and with dignity.
